PLEASE ARRIVE EARLIER IN EACH SESSION AND DO NOT WAIT UNTIL THE LAST SESSION!!  ALL DATES ARE OPEN TO ALL ATHLETES ON ALL TEAMS!!

WE WILL NOT BE OFFERING PHYSICALS IN AUGUST.  IF ANY ATHLETE DOES NOT GET A PHYSICAL ON THESE DATES, YOU MUST SEE YOUR OWN DOCTOR. 

NOTE:  Physicals are good for 12 months.  A health history update form must be completed prior to practice for physicals older than 30 days and must be signed by the school nurse (this is the "PARENT PERMISSION FORM).  Forms may be dropped off at the Athletic District Office beginning August 1st and will be given to the appropriate coach after reviewed and signed by the nurse.
