Working Papers

Students ages 14 to 17 should download the PDF below to obtain working papers.  Please read and follow the directions carefully or your application will not be accepted.  Fill out ONLY part I of the application, then attach the required proof of age and physical exam.  If you have any further questions please call 209-6286 or email OPHSRecords@opschools.org. 

Newspaper Carrier/Farm/Street Trade Working Papers

Child Model/Performer Permits are completed online through the NYS Labor Department.   In some cases they may request you also have your child's school principal complete a form (NY Labor Dept. All working papers documents may be submitted to the OPHS Records Clerk.  The office is located in the Baker Rd. Foyer.
